Registration Guidelines
Registration Checklist
Following is a list briefly stating the requirements for registration in a sanctioned event. All forms, fees and materials must be submitted before teams/players begin play. Read the registration details for additional information.
- All teams must submit an official sanctioned event roster (preferrably online, however event organizer may accept paper rosters on-site).
- All players must sign a USA Ultimate waiver of liability or complete the waiver online for the current year.
- All players must be current USA Ultimate members or pay the one-time event fee.
- Each youth player must complete a Medical Authorization form and turn it in to their chaperone.
- Each team with a youth player must have a chaperone.

Registration Details
Following are USA Ultimate rules regarding team and player registration. Event directors and organizers may set additional rules and deadlines that are specific to their event. All outstanding forms, fees and materials must be submitted before teams/players begin play. Forms can be found at the top of the page.

Team Registration
For each event a team will need the following:
- Completed USA Ultimate sanctioned event roster listing all participating players. (Team contacts must first register their team with the event director and receive confirmation that the team has been accepted into the event before registering the team's roster with USA Ultimate). There are several options for submitting a roster, however the event organzier may require online rostering. Rostering options include:
- Print out the PDF version of the roster, complete it manually and mail it to USA Ultimate or hand it in to the event director.
- Open the Excel version of the roster, complete it electronically and email it to USA Ultimate at sanctioning@usaultimate.org or to the event director.
- Use the online rostering system to submit your roster online. Players' names and USA Ultimate ID#'s are required to do this. College teams at USA Ultimate College Regular Season events must use this option and complete their roster at least 48 hours before the start of the event.
- Teams with youth players must have a chaperone.
Players or teams that fail to register with USA Ultimate will not be covered by USA Ultimate's insurance policy, place the event at risk of losing their sanctioning privileges and place the team/players at risk of not being able to participate in future USA Ultimate events. Keep a copy of your roster on hand. Review the rostering guide for additional instructions.

Player registration
Each player in a sanctioned event must complete the following:
- Each player must be listed on the team roster. You must be added to a roster by your team organizer. See Team Registration details. Or you can register yourself by completing and mailing in the One-time Event and Waiver form found at the top of the page to USA Ultimate in advance of the event.
- Each player must complete a waiver of liability. Waivers are valid for one calendar year (with the exception of lifetime members). Paper waivers can be signed and mailed to USA Ultimate, or waivers can be completed online (players must be logged in to their online account to complete the online waiver).
- Each player must be a current (ie paid) member of USA Ultimate or pay the event fee. There is no team registration fee. Players must sign up as individuals.

- Players that do not want to become USA Ultimate members may pay an event fee to participate. Event fees do not include any membership benefits. Fees are $5 for youth players and $10 for adults. These players can still create a USA Ultimate account online (there is no cost for this) and they can complete the waiver in their online account. If submitting a paper waiver instead of completing the online waiver, please complete the One-Time Event & Waiver form (under the forms listed at the top of the page) to register. The event fee is not available online, a check must be sent to USA Ultimate, or submitted at the event.

Youth Player Requirements
- Youth participants must submit a waiver of liability that has been signed by a parent or guardian (online waivers need to be completed by the participant and their parent/guardian).
- Youth participants must submit a medical authorization form that has been signed by a parent or guardian. Medical authorization forms must be given to the chaperone. These forms should be kept on site throughout the event. They are not submitted to USA Ultimate for sanctioned events.
- An adult that will be responsible for a youth player/s must be designated as the chaperone and must complete a background check for USAU (see: Chaperones). Chaperones at USA Ultimate-sanctioned events must be 21 or older. There is no chaperone to youth ratio requirement, but it is recommended that there be one for every 10 players or two per team. Registration Deadlines
Pre-event Registration
This includes rosters and materials that are submitted prior to the start of the event. Generally these materials are submitted directly to USA Ultimate. Some directors may choose to collect them.
One to two days before the start of an event, a report of everything that has been submitted thus far is sent to the event organizer by USA Ultimate. Any information (rosters, payments, waivers or other forms) that have not been received and processed by this time may not be seen by the organizer. To be sure that your roster, waivers and money are processed on time please make sure they are received by USA Ultimate NO later than 5 days before the start of the event (Mon. 5pm for a tournament beginning on Sat.).
Materials that are not submitted before the event need to be submitted at the start of the event. See late registration for details.

Late Registration (on-site)
This refers to any materials that are submitted at and/or during the course of an event. It is at the discretion of the organizer whether or not to accept registration materials at the event.
If late registration is accepted, all materials must be collected before that team begins playing. Please have these materials ready, labeled and organized.
During late registration, materials may be turned in directly to the event organizer or another staff/volunteer of the event that is designated as the person in charge of registration. Online roster changes can be made until the closing date which is midnight on the final day of the event. After that the roster will no longer be available online.
