Apply for Coverage Through the Insurance Program

Teams, clinics and camps registered with USAU receive insurance coverage, discounts, and more.  Follow these steps if you would like to apply for the insurance program.  As the Event Manager, you must read, understand and agree to comply with the following:

Youth Requirements

 

Mandatory when minors (under 18) are permitted to participate.

 

 

 

 

 

Step 4

 

Application and Contract

 

Complete the application/contract and submit it to USAU along with your deposit.
